• Sign in under the Employees and Returning Customers login
• Your USERNAME is your employee number
• New Employees: Your initial PASSWORD is the last 4 digits of your social security number (You may change your password under the 'My Account' options after you log in if you would like)

If you are **Creating a New Account**:

• Use your EMAIL ADDRESS as your USERNAME when signing up for a new account.

NEW: If you are a *Premier Fidelity Employee*:

• Sign in under the Employees and Returning Customers login
• Your USERNAME is your Premier Health email address
• New Employees: You can request a password reset below to access your account
